Amyloidoses are characterized by the tissue accumulation of misfolded proteins into insoluble fibrils. The two most common types systemic amyloidosis result from deposition immunoglobulin light chains (AL) and wild-type or variant transthyretin (ATTRwt/ATTRv). Cardiac involvement is main determinant outcome in both AL ATTR, cardiac (CA) increasingly recognized as a cause heart failure. In CA, circulating biomarkers important diagnostic tools, allow to refine risk stratification at baseline during follow-up, help tailor therapeutic strategy monitor response treatment. Among amyloid precursors, free established amyloidosis, while plasma assay currently being investigated tool for supporting diagnosis ATTRv predicting novel tetramer stabilizers small interfering RNA drugs ATTR CA. Natriuretic peptides (NPs) troponins consistently elevated patients with Plasma NPs, hold prognostic significance evaluated therapy decision-making value NPs less well established. Biomarkers can be usefully integrated clinical imaging variables all levels algorithm screening prognosis, treatment tailoring.
